Part time, 50 hours per pay period, Day shift
Covenant Medical Group is Covenant Health’s employed and managed medical practice organization, with more than 300 top Physicians and providers spanning the continuum of care in 20 cities throughout East Tennessee. Position Summary:
Provides primary care to patients under the supervision of a physician preceptor, following established medical protocols. Will assist with Transitional Care visits for CMG Primary Care clinics.

Responsibilities
- Maintains established departmental policies and procedures, objectives, quality assurance program and safety standards.
- Provides direct patient care, evaluates outcomes, consults with other specialists as required, and adjusts processes as indicated to ensure optimal patient care.
- Collaborates with physician preceptor to formulate and review medical protocols.
- Obtains complete medical history of the patient, , including both inpatient and outpatient settings to ensure new consults for the structural program are completed in a timely fashion. The Nurse Practitioner will be involved with the post-operative management of the patients in both the inpatient and outpatient settings.
- Identifies health hazards in the patient's environment as well as patient risk factors. Identifies and considers the cultural patterns, economic status and patient resources in plan of care.
- Performs appropriate physical exam as indicated.
- Obtains appropriate laboratory specimens.
- Analyzes objective and subjective data for appropriate diagnosis and treatment.
- Assures treatment and/or plan of care is consistent with established protocols.
- Monitors and ensures accuracy of recording on patient medical records and maintains all required reports, records, statistics, etc. Documents all pertinent information accurately, legibly, and in a timely manner.
- Follows Medicare guidelines for accuracy in CPT and ICD coding.
- Applies the fundamentals of AIDET when communicating with patients.
- Refers patient to appropriate services which exceed scope of practice of the practitioner.
- Provides health promotion counseling and develop patient teaching information.
- Serves as a resource person for other staff personnel. The Nurse Practitioner will lead new initiatives and/or educational programs for both structural and valve pre-operative and post-operative care for both existing and new therapies for our patients.
- Responsible for periodic review of policy and procedures.
- Maintains professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest trends in field of expertise.
- Attends meetings as required and participates on committees as directed. The Nurse Practitioner will also be involved with TVT Registry oversight, STS data entry and other pertinent structural registries with management of staff to include structural program growth.
- Performs other duties as assigned or requested. , including assisting with preparation for structural case reviews, acting as a liaison between cardiology, cardiothoracic surgery, anesthesia, and other specialties involved in the care of structural patients. The Nurse Practitioner will also serve as liaison for structural and valve leadership with the hospital and nursing administration team.
- Collaborate with the patient, physician and other care team members as part of a team based approach to overall patient care.
- Receives incoming referrals from physicians and hospitals and responds quickly to ensure rapid access into our structural system.
- Assists with outreach education in collaboration with supervising physician for development of a structural heart and valve foundation which will be a mechanism to enhance the delivery of structural heart and valve therapies for our patients.
- Educates patient and families regarding the referral process, steps of evaluation, and keeps patient and family informed throughout the structural process in conjunction with the structural heart and valve coordinator.
- The Structural NP may provide coverage for a non-structural NP within the practice, in times of illness, turnover, etc. Coverage may include and is not limited to; clinic visits, on-call coverage, and/or inpatients consults.
- Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives and participates in quality improvement initiatives as requested.

Qualifications
Minimum Education:
None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/ or certification as required by issuing authority.
Minimum Experience:
One (1) year experience. Nurse Practitioner licensed in the state of TN. Minimal travel may be required.
Licensure Requirements:
Must have and maintain certification by American Nurses Association or American Academy of Nurse Practitioners as a Family Nurse Practitioner. Must meet the State's requirements as a Nurse Practitioner. CPR required.
