BASIC PURPOSE OF THE JOB
Provides individualized diabetes self-management education/training to parents, children, and care-givers according to the Scope of Practice, Standards of Practice, and Standards of Professional Performance for Diabetes Educators.

JOB REQUIREMENTS
Supervisory Responsibilities: Yes
Minimum Education: Bachelors Preferred
Degree: Degree in Nursing.
License/Certification Required: TN or Compact RN Licensure Required; Certification as Diabetes Educator required, or as soon as eligible.
Minimum Work Experience: 3 years professional nursing experience required; 1 year experience working with diabetes patients desired.

DUTIES AND RESPONSIBILITIES
- Consider the quality of the work tasks required and how the quality of performance impacts the program, function or care this position must demonstrate.
- Collects DSME/T program participant assessment data, using the AADE7 framework, in a collaborative and ongoing manner and collaboratively develops educational goals, learning objectives, and a plan for educational content and teaching methods with DSME/T child and caregivers.
- Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods and evaluates effectiveness of educational services provided by measuring attainment of learning objectives.
- Documents assessment data, educational plan, education services provided and evaluation results in each child’s medical record and utilizes a team approach to provide services and collaborates and communicates with team members when needed.
- Identifies when a child’s needs are outside the scope of the instructor’s practice and expertise, plus arranges for additional services to meet needs; including but not limited to making referrals to other disciplines and maintaining contact to enhance the continuity of care.
- Contributes to and participates in a continuous quality improvement process to measure DSME/T program and to identify and address opportunities for improvement.

- Coordinates educational initiatives and activities with department directors, leaders, and necessary stakeholders; establishes collegial partnerships contributing to the professional development of peers, students, colleagues, and others; and Advocates for the protection, rights, and opportunities of learners.
- Consider how this position should contribute their unique strengths and abilities to the team meeting its goals as well as how they should utilize the strengths and abilities of their co-workers to meet the goals for the department and their specific position.
- Maintains current knowledge and competency in diabetes practice through participation in continuing education activities. Improve clinical diabetes knowledge and increases the awareness of professional issues.
- Participates in self evaluation of his/her own practice in relation to professional practice standards and guidelines through activities such as peer review and solicitation of feedback from learners and colleagues. Identifies areas of strength and areas for improvement and develops a plan for improvement and growth.
- Collaborates with inter-professional teams, leaders, stakeholders, and others to facilitate positive outcomes for customers.
- Willingly accepts any other assignment that may be requested.

PHYSICAL REQUIREMENTS

- Heavy lifting, pushing, and pulling is required up to 50 lbs occasionally, 20-30 lbs frequently, and 10lbs constantly to move objects of up to 150 lbs. Sudden emergency situations have the potential for exposure to lifting or moving of up to 150 lbs. Frequent bending, walking, sitting squatting, reaching, and standing are required. Keyboard/computer use and/or repetitive motions may be required.
